We are seeking a board-certified or board-eligible primary care physician to join an independent practice in our integrated specialty network. You will deliver longitudinal adult primary care through a blend of in-person visits and secure telehealth, coordinate referrals and transitions with infectious disease and other specialists, and help close care gaps for patients in underserved and rural communities across Georgia.

Position summary

We are seeking a dynamic and dedicated Infectious Disease Advanced Practice Provider — PA-C or NP — to join our innovative healthcare team. In this role, you will deliver expert infectious disease care across inpatient, telehealth, and post-acute settings, collaborate with multidisciplinary teams, and develop individualized, evidence-based treatment plans for patients with complex infectious conditions.

The APP will also support physician-led Antimicrobial Stewardship Program (ASP) activities at our partner hospitals — assisting with data interpretation, reinforcing stewardship recommendations at the bedside, and contributing to ongoing efforts to optimize antimicrobial use and combat resistance. This position offers an exciting opportunity to practice at the top of your license in a fast-paced, patient-centered environment committed to clinical excellence, innovation, and compassionate care.

Hospital consultation

In-person, high-acuity ID consultations across partner facilities in the assigned service area. You will round on hospitalized patients, perform comprehensive assessments including history, physical examination, and vital sign evaluation, and collaborate with hospitalist and specialist teams on complex cases.

Telehealth — virtual

Virtual clinic & follow-up

Telehealth-based outpatient visits for continuity of care, OPAT monitoring, antimicrobial stewardship post-discharge, and chronic ID conditions including HIV, HCV, and HBV. Conducted via secure telemedicine platforms with accurate EHR documentation in compliance with HIPAA.

OPAT & home health agency coordination — integrated throughout the role
  • Serve as the clinical liaison between our practice and home health agencies administering IV antibiotics post-discharge
  • Review and approve home infusion orders, monitor lab results (drug levels, renal function, CBC), and adjust antibiotic regimens remotely
  • Communicate directly with home health nurses regarding patient tolerance, line complications, and clinical changes
  • Conduct telehealth OPAT check-in visits to assess treatment response and prevent unnecessary readmissions
  • Coordinate transitions from inpatient to home infusion to oral step-down therapy across the care continuum
Antimicrobial stewardship support — in collaboration with physician ASP leadership
  • Support physician-led ASP activities at assigned partner hospitals, serving as an engaged clinical partner in stewardship rounds and case reviews
  • Assist with interpretation of ASP data including days of therapy (DOT), defined daily doses (DDD), antibiogram trends, and drug utilization reports to identify prescribing opportunities
  • Reinforce stewardship recommendations at the bedside — supporting de-escalation, IV-to-oral conversion, and dose optimization in collaboration with the supervising ID physician
  • Assist in implementing process change initiatives in antimicrobial control, including rollout of facility-specific treatment guidelines and clinician education efforts
  • Collaborate with hospital pharmacy, infection control, and quality teams as directed by the supervising physician to monitor resistance patterns and contribute to stewardship reporting

The APP will be responsible for servicing an assigned geographic area which may involve visiting multiple facilities within a given day. Candidates must be comfortable with regular travel within the Canton and Jasper, Georgia service area as a routine expectation of this role.
